Job summary

Qualifications

Responsibilities

• Conduct outreach to newly enrolled Medicaid members to complete health screenings.
• Identify medical, behavioral, functional, and social needs.
• Document screening results and outreach in required systems.
• Explain benefits, services, and access to care.
• Coordinate with providers, programs, and community resources.
• Maintain HIPAA compliance and data privacy.
