Job summary

ND Health and Human Services (Adult & Aging Services) seeks a Home and Community Based Services (HCBS) Case Manager to support elderly and disabled individuals in Jamestown, Bismarck, Fargo, and surrounding ND areas. The role involves in-home assessments, care plan development, and coordination with community partners.

The position supports a blended work model, including telecommuting and on-site visits, with travel across ND to serve clients in their homes or facilities.

Qualifications

• Requires LBSW licensure by the ND Board of Social Work Examiners or RN/BSN licensure by the ND Board of Nursing.
• One year of professional human services work experience in relevant roles or a MSW with licensure.
• ND Social Work licensure exam applicants may be considered if awaiting licensure.

Responsibilities

• Provide case management to elderly and physically disabled individuals.
• Perform assessments and develop person-centered care plans.
• Monitor ongoing case management and make referrals to community partners.
• Maintain records and documentation; travel to client homes or facilities for on-site assessments.
