Qualifications

• Verbal and written communication skills.
• Analytical experience with medical data.
• Proficiency in Word, Excel, and PowerPoint.

Responsabilités

• Provide care coordination to members via telephonic and/or field-based care management.
• Inform and educate members on health care programs to address personal health needs.
• Complete member health screenings, comprehensive health assessments, and care planning duties.
