Job summary

Health Care Service Corporation (HCSC) is seeking a position that contacts members and community partners to support healthcare initiatives and campaigns, including scheduling care manager and member appointments and addressing health-related social needs. The role supports clinicians and coordinates with community organizations to design outreach programs.

Responsibilities may include field-based work and managing low-risk members’ health-related needs, education, and outreach screenings.

Qualifications

• Bachelor Degree OR 1 year of college and 3 years health, social service or community experience, OR 4 years' health, social services or community experience.
• Knowledge of medical terminology
• Experience coordinating member related needs, providing assistance to members, and analyzing member needs
• PC proficiency including Microsoft Office applications
• Customer service skills
• Verbal and written communications skills including developing written correspondence to members and to other department personnel and coaching skills, including motivational interviewing, to educate members on medical and BH issues

Responsibilities

• Contact members and community partners to support healthcare initiatives and activities.
• Assist in scheduling care manager and member appointments.
• Support health-related social needs assessments and inquiries.
• Assist clinicians in care management activities and member outreach.
• Coordinate with community-based organizations and community health outreach programs.
• Field-based work may be required.
