Job summary

Total Life, Inc. is launching a telehealth Longevity Program focused on healthy aging, offering comprehensive geriatric assessments, chronic disease management, cognitive health, and GLP-1 therapy.

You will deliver initial longevity assessments and ongoing medical follow-ups to older adults via telehealth, with strong clinical support from our geriatrician CMO. We value flexible scheduling, robust training, and meaningful work serving underserved older adults; all work is performed remotely with

Qualifications

• Active, unrestricted NP license in New York.
• Board certification as an Adult-Gerontology NP (AGNP/AGPCNP) or Family NP with geriatric focus.
• Active DEA registration.
• Experience with Medicare documentation and E/M coding.
• Comfort with telehealth and EHR documentation.
• Reliable high-speed internet and a private, HIPAA‑compliant workspace.

Responsibilities

• Conduct comprehensive initial longevity assessments (60 min) covering functional status, cognition, nutrition, mobility, and fall risk.
• Provide medical follow-up visits for chronic disease management and ongoing care.
• Perform cognitive screening and staging; elevate for further assessment where indicated.
• Prescribe and manage GLP‑1 therapy for appropriate patients.
• Conduct advance care planning conversations.
• Document in our EHR to support accurate Medicare billing.
