Job summary
Total Life is seeking a licensed physician to provide remote cognitive assessment and care plan services for Medicare-eligible seniors. The role emphasizes interpreting test results, creating care plans, and coordinating with healthcare teams in a telehealth setting.
Ideal candidates will have an MD/DO with an active license, experience with MoCA/MMSE, and familiarity with CMS guidelines for cognitive assessments.
Qualifications
- MD/DO with an active, unrestricted medical license.
- Minimum 2 years experience in neurology, geriatrics, psychiatry, primary care, or related field preferred.
- Familiarity with cognitive testing tools such as MoCA, MMSE or equivalents.
- Comfort with telehealth platforms and electronic health records (EHR).
- Must pass a criminal background check and have experience with clients age 65+.
- Active NPI and ability to start with at least 5 hours per week.
Responsibilities
- Interpret cognitive assessment tests remotely for Medicare-eligible individuals.
- Develop and document personalized care plans based on assessment results.
- Ensure compliance with Medicare billing requirements for cognitive assessment services.
- Collaborate with behavioral health professionals to support cognitive care.
- Maintain accurate documentation in an electronic health records system (EHR).
