Job summary

Centene is seeking a Navigator to develop and coordinate care management activities for members, educating families on options to improve access to high-quality care. The role involves collaboration with care teams, tracking progress, and ensuring timely services within Illinois counties.

Requires a Bachelor’s degree and 2–4 years of related experience; CWEL preferred for certain roles. Remote/hybrid work with occasional on-site visits supporting DCFS staff and stakeholders.

Qualifications

• Bachelor’s degree or equivalent experience required.
• Graduate from accredited nursing program if clinically licensed.

Responsibilities

• Develops and coordinates member care plans to achieve quality outcomes.
• Educates members and families on services and benefit options.
• Coordinate between members, families, and care teams to ensure timely access to services.
• Document and maintain member information in compliance with regulations.
• Provide psychosocial support and connect to local resources.
• Perform on-site visits as needed and support care management activities.
