Job summary

Centene is seeking a remote Care Management professional in Missouri to assess and coordinate care for foster care members, developing personalized care plans and educating families on available services.

Requires a Bachelor’s degree with 2-4 years of related experience; nursing licensure preferred. Flexible schedule, with options for remote, hybrid, field, or office work arrangements.

Qualifications

• Requires a Bachelor’s degree and 2-4 years of related experience.
• Graduate from an Accredited School of Nursing if holding clinical licensure.
• Or equivalent experience through accomplishments of applicable knowledge, duties, scope and skill

Responsibilities

• Evaluates the needs of the member, barriers to care, and resources to coordinate the best plan.
• Develops or contributes to personalized care plans and educates members and families on services.
• Coordinates between member/family and care team to ensure access to services.
• Collects and maintains member information to ensure regulatory compliance.
• Performs on-site visits to assess needs and collaborates with providers as needed.

Education

Bachelor’s degree and 2-4 years related experience

Graduate from Accredited School of Nursing when clinically licensed
