Job summary

Centene is seeking a Navigator to develop and coordinate care management for members across Illinois. The role focuses on assessing needs, building personalized care plans, and connecting members to local resources with a strong emphasis on advocacy and family education.

The position is primarily remote (75%), with travel to DCFS/CWCA offices in Illinois and weekend/evening coverage as needed. A Bachelor’s degree and nursing license are preferred where applicable.

Qualifications

• Bachelor’s degree and 2–4 years related experience.
• Nursing license if clinically licensed.
• Willingness to travel within Illinois counties as required.

Responsibilities

• Assess member needs, barriers to care, resources, and recommend/coordinate the care plan.
• Develop personalized care plans and identify providers, specialists, and community resources.
• Provide psychosocial and resource support to members and caregivers.
• Coordinate between members, families, and care team to ensure timely access to services.
• Monitor progress toward plan goals and collaborate with providers for revisions.
• Document and maintain member information to ensure regulatory compliance.
• May perform on-site visits and educate staff and caregivers.
