Position Overview: The LTSS Service Care Manager will manage a caseload of healthcare members with Long-Term Support and Services (LTSS) needs. This member-facing position combines remote administrative work with extensive field work and requires regular face-to-face member visits, assessments, care coordination, service monitoring, and communication with members, caregivers, providers, physicians, discharge planners, and other healthcare professionals.
Training schedule is Monday through Friday, 8:00 AM to 5:00 PM EST. Training and learning journey is approximately 3.5 weeks. Camera must remain on during training. Candidates must be available for the entire training period. No time off is permitted during the 3.5-week training period. Candidates must understand that they cannot miss training.
This is a member-facing field position. Approximately 80% of the role involves travel. In-person visits are generally scheduled 3 to 4 days per week. Remote administrative work is generally scheduled 1 to 2 days per week. Field workload may range from 1 to 4 visits per day. Longer drive times may result in only 1 member visit being completed that day. Face-to-face member visits may require travel up to 2.5 hours one way from the candidate's home residence. Mileage is reimbursed at $0.725 per mile. Lodging is reimbursed when needed. Candidates must have a valid driver's license and be comfortable with the required drive time.
The role is primarily remote with required field work. Once per quarter, the candidate will be asked to attend an in-person team meeting or training session. Virtual team meetings require cameras to be on. Available office locations include Raleigh and Charlotte for applicable quarterly meetings or training.
This position requires significant independent field work and travel. The role involves direct interaction with healthcare members. Candidates should be comfortable conducting assessments in members' homes or other appropriate settings. The position requires regular communication with members, caregivers, providers, physicians, and healthcare partners. Candidates should be prepared for variable daily field travel depending on member locations. Mileage and lodging, when needed, are reimbursed according to the stated reimbursement policy. The position requires completion of the full 3.5-week training period without time off. The role offers the possibility of assignment extension or conversion to permanent employment based on business needs.
