Job summary

CVS Health Corporation is seeking a Virtual Primary Care Nurse Practitioner to join a dedicated care team and deliver evidence-based, primary care for adults 18 and older. This 100% remote, full-time position partners with the NP daily to drive better outcomes for the patient panel.

Ideal candidates are FNP licensed in Western states, hold National Board Certification in Family Practice, and have 5+ years of recent primary care experience.

Qualifications

• Active APRN license in good standing
• Current National Board Certification, Family Practice
• 5+ years of recent Primary Care experience as a Nurse Practitioner
• Ability to work remotely in a care team culture
• Strong communication and organizational skills

Responsibilities

• Demonstrate knowledge of etiologies, risk factors, and pathophysiology of medical conditions across primary, acute, chronic care
• Perform history and physical assessments suitable for age and clinical issue
• Evaluate and treat patients' medical needs with safe, high-quality care
• Counsel and educate patients and families
• Document patient care in the EMR per policies
• Collaborate with care team to coordinate patient care
• Provide patient-centered care for health promotion and disease prevention
• Refer to specialists as appropriate
• Contribute to quality measurement in chronic disease management
• Document patients expressing desire to join the care team panel
• Participate in/lead practice committees such as Epic Optimization or Care Pathways
