Job summary

CVS Health Corporation is seeking a Virtual Primary Care Nurse Practitioner to join a remote care team. The NP will provide evidence-based primary care for adults 18+, collaborating with the care team to ensure safe, high-quality outcomes.

The ideal candidate holds an active APRN license with National Board Certification (Family Practice), at least five years of primary care experience, and the ability to work remotely within a team-based care model across states.

Qualifications

• Active APRN license in good standing.
• Current National Board Certification, Family Practice.
• 5+ years of recent Primary Care experience as a Nurse Practitioner in family medicine or primary care.
• Effective verbal, written, and electronic communication skills.
• Outstanding organizational skills and ability to multi-task.
• Initiative, problem solving ability, adaptability and flexibility.
• Ability to work remotely in a care team culture.
• Proficient with information management and technology.
• Obtain and maintain multi-state licensures.
• Take after-hours and/or overnight call to support patient care.

Responsibilities

• Collaborate with a dedicated care team to provide evidence-based primary care for adults 18+.
• Perform assessments, diagnoses, treatment and disposition planning.
• Counsel and educate patients and families.
• Document care in an electronic health record per policy.
• Coordinate care with internal and external colleagues for quality outcomes.
• Participate in practice committees and Epic optimization initiatives.
