Qualifications

• Board Certified as a Family Nurse Practitioner
• Active Family Nurse Practitioner license in Louisiana
• Consistently professional, confidential, and compassionate
• Willingness to work in a fast-moving, problem-solving environment
• Technologically savvy with charting and HIPAA-secure messaging
• Strong written and verbal communication skills
• Ability to work independently or in a team
• Pass national background check and license search

Responsibilities

• Review caseload and prepare tools to service patients.
• Navigate schedule and coordinate with in-home Sprinters for visits.
• Establish rapport with diverse patients and perform services.
• Conduct health risk assessments and medication reviews.
• Provide care coordination, preventive care interventions, and referrals.
