Qualifications

• Board Certified as a Family Nurse Practitioner (FNP).
• Active California FNP license.

Responsibilities

• Review caseload at start of day and prepare required tools.
• Navigate schedule with in-home clinical team for patient visits.
• Perform health risk assessments, medication reviews, and cognitive screenings.
• Collect information and conduct wellness services during in-home visits.
