Responsibilities

• Perform virtual wellness visits and in‑home assessments as part of care delivery.

• Collaborate with sprinters and in‑home teams to coordinate care.
• Provide patient education and empower self‑care during visits.
• Ensure accurate documentation and HIPAA compliance at all times.
