The Gardens at West Shore in Camp Hill, PA is seeking a Registered Dietitian to implement, coordinate, and evaluate medical nutrition therapy for residents. The role involves education for residents and families and supporting the facility’s nutrition services.

The position offers a full-time schedule with two remote days per week and a compensation rate of up to $45 per hour plus a $5K sign-on bonus. The successful candidate will collaborate with the interdisciplinary team to enhance resident

Qualifications

• Licensed Registered Dietitian (RD) required.
• Minimum of two years of experience as a dietitian in long-term care, or one year as a consulting dietitian in long-term care.
• Strong communication, organizational, and clinical assessment skills.
• Commitment to providing individualized, resident-centered nutrition care.

Responsabilités

• Conduct preliminary and comprehensive dietary assessments for residents.
• Develop individualized dietary care plans, identifying goals and resident needs.
• Engage residents and families in creating and reviewing care plans.
• Ensure dietary staff understand and utilize care plans in daily service delivery.
• Identify and recommend appropriate medical nutrition therapy based on resident medical records.
• Ensure care plans include necessary equipment or utensils to meet residents’ nutritional needs.
• Collaborate with the interdisciplinary team to support resident health and well-being.
