Department of Health and Social Services (DHSS) in Delaware seeks a Health/Human Service Case Manager I to determine initial and ongoing eligibility for SNAP, Medicaid, CHIP, childcare, and GA. You will also refer for benefits not provided by DSS and assist with counseling and referrals.

You will work with diverse clients, collaborate with hospitals and community partners, document outcomes, and use information systems to track progress. Telework is available under state policy.

Competenze

• Associate's degree or higher in Behavioral or Social Science or a related field.
• Three years’ experience in health or human services support such as interviewing clients and assessing needs.
• Six months’ experience in making recommendations as part of a client’s service plan.
• Six months’ experience in using an automated information system to enter, update, modify, delete, retrieve/inquire and report on data.

• Interviews clients, family members, and others to determine eligibility and connect clients with health and human services.
• Develops a service plan with hospitals, clinics, agencies and resources to aid clients.
• Evaluates information from interviews and refers to proper channels for review.
• Provides guidance and relevant information to clients and families prior to receiving requested service.
• Coordinates with other agencies, hospitals, clinics, and resources to expedite the process for the client.
• Prepares case histories and reports and maintains records and logs of case activities.
• Uses automated information systems to research, review, evaluate, and report on health/human service data.
• Communicates effectively with people across cultures and backgrounds.
