Job summary

Delaware Department of Health and Social Services (DHSS) seeks a Health/Human Service Case Manager II to determine initial and ongoing eligibility for SNAP, Medical Assistance, CHIP, Subsidized Childcare, GA, and TANF, and to coordinate with TANF and SNAP vendors.

You will counsel, case manage, and refer for benefits not provided by DSS, maintain records, develop service plans, monitor delivery, and connect clients with needed services.

Qualifications

• Six months’ experience in case management.
• Six months’ experience in making recommendations for benefits.
• Six months’ experience in interpreting laws, rules, and procedures.
• Six months’ experience using automated information systems for data handling.
• Six months’ experience in narrative report writing.

Responsibilities

• Interviews clients to determine needs for health and human services.
• Develops cost‑effective service plans using in‑house and contracted providers.
• Monitors service delivery and authorizes changes as needed.
• Maintains case records and required forms with accuracy.
• Provides guidance and connects clients with needed services.
