Job summary

Department of Health and Social Services (DHSS) in Delaware seeks a Health/Human Service Case Manager II to determine initial and ongoing eligibility for SNAP, Medicaid, CHIP, GA, TANF and related benefits. Qualifications

• Six months' experience in case management with assessing, planning and evaluating options to meet human service needs.
• Experience making recommendations as part of a client’s service plan (clinical treatment, counseling, or determining eligibility).
• Six months' experience interpreting laws, rules, regulations, standards, policies, and procedures.
• Six months' experience using an automated information system to enter, update, modify, retrieve and report data.
• Six months' experience in narrative report writing.

Responsibilities

• Interviews client, family, or professional sources to determine needs for health and human services.
• Develops comprehensive service plans utilizing in-house and contracted providers.
• Conducts ongoing monitoring and assessment of service delivery and adjusts as needed.
• Maintains case records, reports, and required forms.
• Establishes effective relationships with clients, family, community, and professionals.
• Provides information to connect clients with needed services and referrals.
