Job summary

Community Health Network is seeking a Director of Clinical Delivery for IHCI based in Indianapolis. You will provide strategic and operational leadership for clinical care management services, overseeing nursing, social work, medical assistants, and care management teams across multiple markets.

The role drives population health initiatives, value-based care performance, clinical quality outcomes, patient and provider satisfaction, and operational excellence.

Qualifications

• Bachelor’s degree in Nursing or Social Work or related field required.
• Five or more years of clinical staff management in population health or managed care, or hospital case management.
• Five or more years of operational management in a clinical setting.
• Indiana RN license if nurse, with NLC eligibility.
• Strong communication and relationship-building skills.
• Willingness to travel between central and southern Indiana markets.

Responsibilities

• Lead day-to-day and strategic clinical operations for care management programs.
• Oversee RNs, Social Workers, Medical Assistants, and care management staff.
• Drive population health and value-based care initiatives.
• Collaborate with physicians, health systems, and executives to improve quality and financial performance.
• Monitor program outcomes and identify opportunities for improvement.
• Lead change management and clinical transformation initiatives.
• Manage budgets, staffing plans, and operational performance.
