The Innovative Healthcare Collaborative of Indiana, (IHCI), is a joint venture between Community Health Network and Deaconess Health System. Its goal is to support our sponsors and partners in their strategic evolution to positively impact and improve the healthcare delivery system.
The Population Health Advisor, IHCI, has responsibility for driving value-based care initiatives in Deaconess’s regional practices near and around Evansville, Indiana, Southern Illinois or Central Kentucky. This position works closely with the Quality and Analytics teams and helps integrate clinical programs, network saving initiatives, quality initiatives, and resources into the practice workflows with the intent to optimize the patient experience and positively impact physician engagement and quality of care provided. This role supports operational functions essential to IHCI’s success in value-based contracts, focusing on closing quality care gaps, presenting insights to providers and health system leaders, and advancing network savings initiatives. The ideal candidate is relationship-driven, analytical, adaptable, and passionate about improving patient and provider experience.
Applicants for this position should be able to communicate effectively with internal team members, and key stakeholders - including public speaking, PowerPoint, dashboards, visualizations, and/or other presentation formats. Applicants must be highly organized and self-motivated individuals with the ability to adapt to various workspaces and work autonomously. This applicant has a collaborative work style with the ability to work across different teams, areas of expertise, and adapt to ambiguous environments and clientele.
