Job summary

Galileo Health is seeking a Nurse Practitioner to deliver digital-first, asynchronous chat and synchronous video-based primary care. You will join a collaborative clinical care team and leverage data, technology, and rigorous processes to achieve excellent patient outcomes with personalized care and shared decision making.

The role emphasizes complex and chronic care management across all ages, with a proactive, patient-centered approach and a strong emphasis on evidence-based practice.

Qualifications

• Have 5+ years of advanced clinical experience in complex care, chronic care management, or primary care.
• Are experienced in or have an appetite for learning digital-first healthcare (e.g. virtual medicine).
• Board certified as a Nurse Practitioner and have MA licensure (multi-state preferred).
• Proficiency with written and spoken medical Spanish is a nice to have.
• Comfortable with using technology and various applications to deliver care virtually (platforms, Google Suite, Slack).

Responsibilities

• Provide best-in-class care for Galileo patients managing complex and chronic illnesses through our app and technology and your clinical judgement.
• Provide compassionate and empathetic care to patients from age 0-100 across all needs, including chronic conditions.
• Work with a diverse patient population and contribute to an inclusive culture.
• Collaborate with team members to solve patient problems efficiently with resourcefulness and a can-do attitude.
