Advocate Health invites applications for a full-time Dietitian/Nutrition Care position based in the Southpark Diabetes Clinic. The role focuses on medical nutrition therapy, patient education, and interdisciplinary collaboration with physicians and nursing teams.

Eligible for a sign-on bonus; remote work possible for candidates in listed states. The position requires RD licensure or RD/LDN eligibility with timely registration, a relevant bachelor’s (or higher) degree, and potential specialty

Competenze

Bachelor's degree in dietetics required for RD status.
RD/LDN registry eligible; must become RD registered within 4 months of hire.
Specialty certifications (e.g., CDCES, CNSC, AND) may be required.

Mansioni

Assess and monitor nutritional status; adjust care using current literature.
Provide medical nutrition therapy and counseling; complete consults and referrals.
Coordinate nutrition into total patient therapy with physicians and nurses.
Document care plans, assessments, consultations, and analyses timely and accurately.
Develop and deliver patient education; tailor materials to learner level and needs.
Lead in-services for team members and trainees; mentor dietetic interns as applicable.
Educational dissemination of nutrition findings to medical teams and patients.
Collaborate across departments to ensure standards of excellence and patient satisfaction.
