Job summary
Advocate Health's Diabetes and Nutrition Care at Southpark seeks a full-time clinician to deliver medical nutrition therapy and counseling for patients, including remote weight management. Qualifications
- Assess and monitor nutritional status and adjust care using current literature.
- Provide MNT/counseling including assessment and education.
- Coordinate with physicians and nurses to integrate nutrition into care.
- RD registry eligible or licensed by state Board of Dietetics.
Responsibilities
- Document nutrition care plans, assessments and education in required tools promptly.
- Develop, evaluate and utilize education materials tailored to learner level.
- Mentor dietetic interns and residents; contribute to team training.
- Share nutrition research findings with medical team and patients.
