Job summary

Advocate Health is seeking a registered dietitian to provide medical nutrition therapy and counseling for patients at the Southpark Diabetes Clinic. The role involves assessing nutritional status, developing care plans, and coordinating with the medical team to integrate nutrition into overall treatment.

Remote weight management options may apply. Eligible for sign-on bonus; position is 40 hours per week, Monday–Friday, 8 am–5 pm.

Qualifications

• Bachelor's degree required or RD eligible depending on credentialing timeline.
• RD/LDN registration eligible; must be registered within 4 months of hire.

Responsibilities

• Assesses and monitors nutritional status and adjusts care based on current literature.
• Provides medical nutrition therapy and counseling, completes consultations and referrals.
• Coordinates nutrition care with physicians and nurses as part of the total therapy program.
• Documents nutritional care plans, assessments, consultations, and education.
