Advocate Health is seeking a Registered Dietitian to provide medical nutrition therapy within the Southpark Diabetes Clinic as part of the GCMG Diabetes and Nutrition Care team. The role supports remote weight management and requires RD licensure with eligibility for state practice, with a 40-hour work week and a 8am-5pm schedule, Monday through Friday.

Base pay ranges from $28.55 to $42.85 per hour with a sign-on bonus eligibility and comprehensive benefits.

Qualifikationen

• Bachelor's degree if RD before 1/1/2024; Master's degree if RD on or after 1/1/2024 required.
• RD/LDN registry eligible or Registered Dietitian licensure required by applicable state Board.
• Specialty certifications (CDCES, CNSC, AND Certifications) may be required.

Aufgaben

• Assesses and monitors nutritional status and adjusts care per literature and patient data.
• Provides MNT/counseling including assessment, education, and referrals.
• Coordinates nutrition with physicians, nurses, and the care team.
• Documents nutritional care plans, assessments, and education in required tools timely.
• Provides/ develops patient education materials and assesses learning needs.
• Performs in-service education for team members and mentors dietetic interns.
• Teaches and disseminates nutrition research findings to the medical team and patients.
• Communicates with multidisciplinary departments to ensure care standards are met.
