Job summary

Advocate Health is seeking a Full-time Registered Dietitian (RD) to assist in the Southpark Diabetes Clinic, delivering medical nutrition therapy and nutrition care planning. This role includes remote weight management responsibilities and a standard 8:00 am–5:00 pm, Monday–Friday schedule with a sign-on bonus eligibility.

The role requires RD licensure (or RD/LDN registry eligibility) and documented ability to coordinate nutrition into comprehensive patient care, with responsive documentation

Qualifications

• Bachelor's degree in Dietetics or equivalent
• licensure as Registered Dietitian (RD) or RD/LDN registry eligible; licensed by state Board of Dietetics and Nutrition

Responsibilities

• Assess and monitor nutritional status of patients and adjust care per current guidelines.
• Provide medical nutrition therapy (MNT) including assessment, education, and counseling.
• Document care plans, assessments, and education in required tools timely and thoroughly.
• Develop and evaluate educational materials; tailor learning to learner level and needs.
• Mentor dietetic interns and staff; participate in team education and sharing of nutrition findings.
