Job summary

Advocate Health seeks a Dietitian for a remote weight management program within the Diabetes and Nutrition Care team. The role conducts nutrition assessments, provides medical nutrition therapy, and coordinates care with physicians and nursing staff.

Scheduling is Monday–Friday, 8 am–5 pm, with sign-on bonus eligibility. Requires RD licensure (or registry-eligible) and a relevant degree; remote work is available to eligible states.

Qualifications

• Assesses and monitors nutritional status of patients/clients and adjusts nutritional care using current literature.
• Provides MNT/counseling including assessment, education, development, monitoring, and implementation.
• Communicates with physicians, nurses and others to coordinate nutrition into total patient care.
• Completes all required documentation including nutrition plans, assessments, and education in required tools.
• Under supervision, participates in orientation, training, and mentoring of team members and dietetic interns.

Responsibilities

• Teaches and disseminates nutrition research to medical teams and patients.
• Delivers education materials and tailors content to learner development and level of understanding.
• Ensures patient care standards are met and documentation is thorough and timely.
