Advocate Health is seeking a Registered Dietitian to join the Diabetes and Nutrition Care team in Southpark, Maine. This is a full-time role with 40 hours per week, including remote weight management/MNT options where eligible.

Sign-on bonus is available for eligible candidates. The position focuses on medical nutrition therapy, nutrition assessment, and collaboration with the medical team to optimize patient outcomes.

Qualifikationen

• RD credential and state licensure required.
• Bachelor's degree (RD before 1/1/2024); Master's if RD on or after 1/1/2024.
• RD/LDN registry eligible; must obtain RD/LDN registration within 4 months of hire.

Aufgaben

• Assess and monitor nutritional status and adjust care.
• Provide MNT/counseling and develop nutrition plans.
• Coordinate nutrition with physicians and other team members.
• Document care plans, assessments, and education in records.
• Provide/Develop patient education; create teaching materials.
• Participate in in-services and mentor interns/dietetic interns.
• Disseminate nutrition research findings to the medical team.
• Communicate with multidisciplinary departments to maintain standards of excellence.
