Job summary

Advocate Health offers a remote MNT position within the Diabetes and Nutrition Care: Southpark clinic, with a Monday–Friday schedule and 40 hours per week. The role supports weight management services remotely, including counseling and education, with eligibility for a sign-on bonus based on location and credentials.

Applicants must meet RD/LDN requirements and be licensed in the relevant state; compensation is aligned with market rates and benefits are provided.

Qualifications

• Bachelor's degree in Dietetics is required.
• RD/LDN registry eligible or Registered Dietitian must be licensed in the applicable state.
• Specialty certifications may be required (e.g., CDCES, CNSC).

Responsibilities

• Assesses and monitors nutritional status of patients/clients and adjusts care based on current guidelines.
• Provides MNT/counseling, including assessment and education for patients in assigned areas.
• Coordinates nutrition with physicians, nurses and other team members.
• Documents nutritional care plans, assessments, consultations and analyses in required tools.
• Develops and uses educational materials; assesses learner needs and responses.
