Job summary

Advocate Health is seeking a Registered Dietitian for the GCMG Diabetes and Nutrition Care: Southpark clinic in Colorado. The role involves delivering Medical Nutrition Therapy, assessing nutritional needs, and coordinating care with the medical team in a full-time, on-site setting.

Schedule is Monday–Friday 8am–5pm with a sign-on bonus eligibility. Remote weight management options exist; remote candidates must reside in CO among other states listed for remote opportunities.

Qualifications

• Must have RD registration or eligibility; licensure by state required.
• Bachelor's degree if RD before 1/1/2024, Master's if RD on/after 1/1/2024.
• Specialty certifications may be required.

Responsibilities

• Assesses and monitors nutritional status and adjusts care.
• Provides Medical Nutrition Therapy / counseling including assessment and education.
• Coordinates nutrition into total patient therapy with physicians and nurses.
• Completes documentation of care plans, assessments, and nutrition education.
• Assesses education effectiveness and develops materials for patients.
• Provides in-services to team; mentors dietetic interns as applicable.
• Participates in orientation and training under manager or designee.
• Teaches and disseminates nutrition research findings to team and patients.
• Communicates with multidisciplinary departments to ensure standards of excellence.
