Nutrition that Works, LLC is seeking a consultant dietitian to join our team for a long-term care facility in Lake Providence, LA, at 20 hours per month. Enjoy a flexible schedule and excellent pay with some remote options.

The role includes completing clinical nutrition assessments, documenting with the Nutrition Care Process in the EMR, and collaborating with an interdisciplinary care plan team. Requirements include CDR credentialing and dietetic licensure.

Qualifications

• Must be credentialed by the CDR and licensed as a dietitian.
• Strong communication and client-centered approach.
• Experience with care plans and nutrition assessments preferred.
• Familiarity with EMR and the Nutrition Care Process.

Responsibilities

• Complete clinical nutrition assessments.
• Document using the Nutrition Care Process in the EMR system.
• Collaborate with the interdisciplinary care plan team.
• Monthly sanitation audit.
• Calculate TF (therapy/fluids) as applicable.
• Make recommendations for wounds and significant weight changes.
