Job summary

Nutrition that Works, LLC seeks a consultant dietitian to support a Vermont long-term care facility on a remote basis, delivering 8 hours per week. The role focuses on clinical nutrition assessments and Nutrition Care Process documentation.

You will collaborate with the care team, complete MDS Section K, calculate TF, and provide wound and weight-change recommendations while maintaining a patient-centered approach and strong communication skills.
