Job summary

Nutrition that Works, LLC in Lake Providence, LA is seeking a consultant dietitian to join our team for a long-term care facility engagement, approximately 20 hours per month. The role emphasizes clinical nutrition assessments and care planning within a small, collaborative team.

The successful candidate will document using the Nutrition Care Process in the EMR, work with the interdisciplinary care plan team, and perform monthly sanitation audits along with weight change and wound care

Qualifications

• CDR credential required.
• Active dietetic licensure in good standing.
• Positive, client-centered approach with strong communication.

Responsibilities

• Complete clinical nutrition assessments.
• Document using Nutrition Care Process in the EMR.
• Collaborate with interdisciplinary care plan team.
• Monthly sanitation audit.
• Calculate TF and monitor wound and weight changes.
• Provide evidence-based recommendations.
