Job summary

Community Health Plan of Washington is seeking a diabetes education specialist to provide evidence-based education, coaching, and chronic disease support for members in the Diabetes Care Program. The role partners with members, providers, and care teams to improve outcomes through education and behavior change, within professional guidelines.

Required licensure (RN or RD) in Washington, with CDCES or BC-ADM certification preferred or to be obtained within two years.

Qualifications

• Bachelor’s degree in nursing (preferred for nurses) or Master’s degree in nutrition, Dietetics or relevant field, or equivalent.
• Current, unrestricted license in Washington state as RN or RD, or other relevant licensure.
• Certification such as CDCES or BC-ADM required or to be obtained within two years of hire.
• Minimum of two years’ experience in chronic disease management, diabetes education, or health coaching.

Responsibilities

• Provide evidence-based diabetes self-management education and support, chronic disease education, and health coaching using motivational interviewing.
• Assess members' status, health literacy, readiness for change, barriers, and social needs to set individualized goals.
• Educate members on diabetes processes, medications, glucose monitoring, nutrition and activity.
• Interpret lab results and CGM data to tailor education and reinforce treatment goals.
• Provide Medical Nutrition Therapy (MNT) for diabetes and chronic conditions.
• Collaborate with primary care providers and interdisciplinary teams to coordinate care and referrals.
• Document assessments, education, interventions, goals and outcomes in the care management system.
