Job summary

Qualifications

Responsibilities

• Complete screenings, initial treatment plans, risk assessments, and therapy sessions.
• Screening and triaging crisis intervention as directed.
• Document services thoroughly, accurately, and promptly.
• Consult with other agencies and professionals with proper releases of information.
• Attend staff meetings and meet performance expectations.
• Support CC BH Clinic concepts and integrate behavioral with physical health care.
• Travel as needed to transport clients and support services.
