Job summary

Qualifications

Responsibilities

• Conduct screenings, initial treatment plans, and risk assessments for crisis clients.
• Perform crisis triage and provide timely therapy sessions.
• Document services accurately and thoroughly.
• Coordinate with agencies and professionals with proper releases.
• Participate in staff meetings and meet performance expectations.
• Support CCBCHC concepts and integrate behavioral health with physical health care.
• Travel as needed to transport clients.
