Job summary

Heartland Family Service in Iowa seeks a qualified professional to provide crisis intervention and risk assessment within a team environment. The role requires flexible availability for weekday, evening, weekend shifts and the ability to work remotely as part of the agency’s on-call coverage.

You will assess clients, develop intervention plans, coordinate referrals, and maintain documentation while collaborating with community resources to support healthy families and communities.

Qualifications

• Minimum of a Bachelor’s degree in psychology, social work, nursing, education with at least one year of mental health experience or a High School Diploma with two years in human services.

Responsibilities

• Provides crisis intervention and risk assessment for referrals from community, law enforcement, or schools.
• Makes recommendations and referrals to appropriate levels of care following clinical consultation.
• Conducts crisis-focused assessments and documents within 24 hours of crisis call.
• Establishes an intervention plan with client and referral source detailing immediate needs and follow-up.
• Follows up with crisis clients 24 hours post-crisis and maintains a 30-day ongoing caseload.
• Maintains relationships with community resources and provides referrals as needed.
• Identifies client needs and goals for ongoing post-crisis support and services.
• Maintains clinical records meeting timeliness and completeness standards.
