Responsibilities

• Conduct community outreach to build trust, identify needs, and connect individuals to relevant programs and resources
• Engage with clients in a respectful, nonjudgmental manner while promoting participant safety and dignity
• Assess basic service needs and make appropriate referrals to internal programs and community partners
• Maintain accurate records of outreach activity, referrals, and follow-up outcomes in accordance with program requirements
