Qualifications

• Bachelor's degree required or equivalent in a relevant field.
• 3+ years in community health, patient advocacy, or related role.
• Experience navigating healthcare systems and linking clients to resources.
• Proficient with telehealth and digital health records.

Responsibilities

• Conduct virtual needs assessments to identify health and social service requirements.
• Guide clients to access healthcare services, including primary care and mental health support.
• Help schedule medical appointments and explain follow-up care.
• Educate on preventative health and chronic disease management.
