Job summary

Community Action Agency South Central Michigan is seeking a Community Health Worker to conduct outreach in rural areas, perform screenings, and help individuals access health and social services. You will provide education about CA programs, Medicaid benefits, and community resources while building trust and ensuring culturally responsive support.

Responsibilities

• Conduct outreach in rural communities through home visits, events, and partner sites.
• Perform screenings and identify barriers to care; assist with navigation of medical and social services.
• Provide education on CA programs, Medicaid benefits, preventive care, and community resources.
• Maintain and share information to support reporting, evaluation, and program improvement.
• Represent CA in community meetings and collaborate with partners to expand service access.
