CVS Health seeks a Community Health Worker to support Aetna members in Louisiana. This full-time remote position requires extensive in-home outreach, education on care plans, and collaboration with healthcare providers and case managers.

The role focuses on reducing health disparities, improving access, and guiding members through social determinants of health, with travel to member homes and community locations.

Qualifications

• Must reside in LA within Region 9 (St. Tammany, Livingston, Washington, Tangipahoa).
• Complete Certified Community Health Worker/Promotora certification within 12 months of start date.
• Travel 60-75% with reliable transportation.
• 3-5 years of relevant work experience.
• 1-2 years of clinical support experience.
• 1 year of community outreach experience in LA.
• 1 year experience with clients with clinical needs (physical/behavioral health).
• Proficiency with PC and MS Office Suite and EHR systems.
• Flexibility to work outside core hours (Mon-Fri 8am-5pm) and 2 days until 9pm.
• Variable schedule as needed.

Responsabilités

• Conduct home visits to reinforce care plans and adapt tasks to member culture and language.
• Educate providers on social determinants affecting health outcomes.
• Build relationships with members, providers, case managers, and staff from diverse backgrounds.
• Assist with care plan implementation and health/social management planning.
• Help identify socio-economic barriers and connect to community services.
• Assist with medication adherence and reviewing current medications.
• Document in electronic health records and complete program reports.
