Job summary

CVS Health Corporation is seeking a Community Health Worker to support Aetna members in Louisiana. This full-time, remote role requires 60-75% travel within the region, assisting with care plans, education, and care coordination.

You will work under supervision and must complete CHW certification within 12 months if not already certified. Ideal candidates bring 3–5 years of relevant experience, proficiency with MS Office and EHRs, and flexibility to work extended hours as needed.

Qualifications

• Must reside in Louisiana within Region 9.
• Travel 60-75% and have reliable transportation.
• Experience with clinical support and community outreach.

Responsibilities

• Conduct home visits to reinforce care plans and address cultural needs.
• Educate providers on social determinants affecting health outcomes.
• Build relationships with members, providers and case managers.
• Assist with care plan implementation and health management.
• Identify socio-economic barriers and facilitate access to services.
• Document visits and maintain records in EHR systems.
• Travel to client homes and community locations as required.
