Job summary

Health & Human Services Commission (HHSC) in El Paso is seeking a Community Care Worker III to perform moderately complex LTSS and waiver-related duties. You will collaborate with case managers, enroll individuals in Medicaid 1115 waiver programs, and support transitions from nursing facilities to community settings.

The role requires knowledge of CCSE/PSU programs, computer proficiency, and the ability to work under general supervision.

Qualifications

• Knowledge of Medicaid 1115 waiver programs and related policies.
• Strong understanding of LTSS and CCSE processes.
• Experience with enrollment, eligibility, and waiver programs.

Responsibilities

• Provide case management support for LTSS and 1115 waiver services.
• Assist in determining MCO enrollment and waiver eligibility in systems.
• Notify members of waiver program eligibility decisions and deadlines.
• Process fair hearings and manage appeal documentation and timelines.
• Assist with enrollment and transition tasks for nursing facility to community settings.
• Register waiver program eligibility in appropriate systems and coordinate with MEPD staff.
• Support data entry and verification in TIERS, SAS, and related state software.
• Coordinate with CCSE, MCOs, and other staff to ensure timely service delivery.
