Job summary

Health & Human Services Commission in Texas seeks a Community Care Worker III to support Medicaid CHIP services, waiver programs, and enrollment activities in Harlingen. You will coordinate with MCOs, MEPD, and HHSC departments to facilitate community-based care for eligible individuals.

Qualifications

• Requires High School Diploma or GED; preferred case management experience.
• Knowledge of Medicaid waiver programs and enrollment processes is a plus.
• Experience with interacting in complex policy interpretation and regulatory requirements.

Responsibilities

• Attend regular schedule and perform duties as assigned.
• Develop, coordinate, and explain waiver programs to applicants and members; authorize or deny services.
• Validate eligibility and register waiver program eligibility in systems; coordinate enrollments.
• Assist with transitions from nursing facilities to community settings.
• Handle fair hearing processes and monitor outcomes.
• Prepare reports and resolve enrollment errors; support MEPD activities.
