Job summary

Remote Care Partners is seeking a compassionate Nurse Practitioner to join our fully remote virtual care team. You will provide adult primary care via telehealth, conduct RPM and CCM visits, and support weight management programs while leveraging our healthcare technology platform.

The role emphasizes preventive care, collaboration with a multidisciplinary team, adherence to Medicare guidelines, and ongoing professional development in a flexible remote environment.

Qualifications

• Active, unrestricted APRN license in good standing in SC, VA, and/or GA.
• Mississippi and Alabama licensure is a plus.
• Minimum of 3 years NP experience.
• Experience in primary care, chronic disease management, obesity medicine, telehealth, or population health preferred.
• Comfort delivering care through telehealth and documenting in EHR.
• Ability to work independently while collaborating with a multidisciplinary team.

Responsibilities

• Conduct virtual primary care visits for adult patients.
• Perform RPM and CCM visits.
• Conduct weight management consultations and follow-ups.
• Provide ongoing clinical oversight of RPM, CCM, and weight management populations.
• Collaborate with nurses, care coordinators, specialists, pharmacists, and other healthcare professionals to optimize patient outcomes.
• Educate patients on disease prevention, lifestyle modification, medication adherence, and self-management strategies.
• Ensure compliance with Medicare, payer, state, and organizational guidelines.
• Participate in quality improvement initiatives and population health management efforts.
