Qualifications

• MD, DO, NP, PA or equivalent medical degree/licensure required.
• Active coding certification: CRC, CPC, CCS or equivalent.
• Minimum 2 years of medical coding experience, with HCC or risk adjustment coding preferred.
• Working knowledge of ICD-10-CM coding guidelines and CMS HCC models.
• Proficiency with EHR systems and coding software.

Responsibilities

• Review medical records and provider documentation to identify and validate diagnosis codes for HCC and risk adjustment.
• Conduct in-home or virtual health assessments documenting chronic conditions for risk adjustment coding.
• Assign and verify ICD-10-CM codes per guidelines and payer policies.
• Perform chart reviews to identify documentation gaps and improve coding accuracy.
• Collaborate with providers and staff to clarify documentation and ensure accurate condition capture.
• Educate providers on documentation improvements related to risk adjustment coding.
• Monitor coding accuracy via internal audits and QA reviews, identifying trends.
• Stay updated on CMS and payer risk adjustment methodology changes.
• Maintain HIPAA compliance in handling protected health information.
• Support risk adjustment initiatives with quality, analytics, and compliance teams.
