Responsibilities

1. Review medication lists to identify and eliminate barriers to therapy, serving as the member's Pharmacy Navigator to improve medication adherence and prevent adverse events.
2. Assist members in accessing medications by managing refills, prescriptions, appointments, and addressing financial barriers, while documenting care and communicating with providers.
3. Collaborate with Pharmacists and healthcare partners to communicate treatment plans and coordinate MTM interventions.
4. Partner with community pharmacies to monitor and promote medication adherence.
5. Conduct patient health assessments to identify potential barriers to medication adherence, including medication and access issues.
6. Handle member and provider communications via phone or in person as needed.
7. Perform additional duties as assigned.

Minimum Qualifications

• High School Diploma or GED from an accredited institution.
• Experience in clear, professional written and verbal communication.
• Experience communicating with patients, providers, and pharmacists about medication issues.
• Nationally certified Pharmacy Technician (PTCB, NHA) or licensed/registered Pharmacy Technician in good standing.

Preferred Qualifications

• Bilingual skills.
• Medication Therapy Management Certification.
