Duties & Responsibilities

1. Review medication lists to eliminate barriers to therapy and serve as the member's Pharmacy Navigator to ensure medication adherence and minimize gaps and adverse events.
2. Assist members in accessing medications by managing refills, prescriptions, appointments, and addressing financial barriers, while coordinating with providers and documenting care.
3. Collaborate with Pharmacists and healthcare partners to communicate treatment plans and coordinate MTM interventions.
4. Monitor and communicate member medication adherence with community pharmacies.
5. Conduct patient health assessments to identify barriers to medication adherence, considering medication, patient-related factors, and access to care.
6. Handle member and provider communications via telephone or in person if needed.
7. Perform additional duties as necessary.

Minimum Qualifications

• High School Diploma or GED from an accredited institution.
• Experience in clear, professional written and verbal communication.
• Experience communicating with patients, providers, and pharmacists about medications.
• Nationally certified Pharmacy Technician (PTCB, NHA) or registered/licensed Pharmacy Technician in good standing.

Preferred Qualifications

• Bilingual skills.
• Medication Therapy Management Certificate.
