Job summary

Mindoula Health Inc. seeks an experienced Clinical Partnership Manager for its Collaborative Care Program. This remote role, preferably based in Oregon, focuses on activating physicians within contracted practices to refer patients to CCP.

You will manage accounts, drive referral growth, and collaborate with Mindoula clinical teams to address provider needs and program satisfaction. The role requires 3+ years in sales/account management with provider referrals, strong communication, and travel

Qualifications

• Bachelor’s degree in healthcare or business required.
• Three+ years in a sales or account management role with provider referrals experience preferred.
• Travel up to 50% in a high-growth environment.
• Strong project management skills and ability to build partnerships.

Responsibilities

• Grow partnerships within the Collaborative Care Program at account, practice, and provider levels.
• Activate physicians to refer patients to CCP; meet referral volume goals.
• Regularly communicate with accounts to review enrollment, growth opportunities, and satisfaction.
• Create and review referral performance reports with accounts/practices.
• Track engagement and intervene when referral slippage occurs.
• Advise on mitigation plans against industry/account changes.
• Coordinate with ClinOps to escalate clinical concerns when needed.
• Provide re-education to referring providers on CCP referrals.
